Appointments

Forms

To prepare for your visit, new patients are asked to complete several forms in advance.  This will expedite your visit.  New patients are asked to complete a Patient Registration. You may also need to sign a form to have your healthcare records sent to us, or have us send records elsewhere.  In addition, you will be asked to complete a Release of Information Form. For your convenience, we offer different options for completing the forms. You will be sent a link via text message. You can either click on that link and complete the forms on your smart phone, or paste the link into a web browser on your tablet or computer –or– you can print and complete the forms to bring to the office with you. If you are unable to complete the forms online or print, please arrive 20 minutes prior to your appointment to complete these forms in the office.

Payments for Visits

Altrix Primary Care – Nashua offers several payment options for covering the services we provide for you. Option 1 is payment through commercial insurance plans. (See BILLING AND INSURANCE/INSURANCE below). And, of course, you may also pay directly for our services (Option 2).  But we now offer a new option (Option 3) for people without health insurance or those with a high deductible plan: ALTRIX DIRECT. ALTRIX DIRECT is an innovative new model that improves access to high quality care with a simple, affordable monthly or annual membership fee. And there are no deductibles, no co-pays. (See ALTRIX DIRECT-patients for more information).

For all patients, any copays, coinsurances, deductibles, noncovered services and self-pay balances will be collected at the time of service.

Billing and Insurance

We are sensitive to your concerns about healthcare costs and share a desire to keep charges to a minimum.  Our first and foremost desire, however, is to provide you the highest quality of care. If at any time you have concerns regarding the medical necessity or cost of a recommended test or treatment, please talk with us.

Insurance and billing procedures can often be complex and confusing. To help you take full advantage of your benefits, we have an experienced office manager who will guide you. Please call us with your billing or insurance- related questions at 603-821-4009.

Our nurse practitioners are contracted primary care providers with many insurance plans. In today’s confusing insurance environment it is important for you to be knowledgeable about the requirements of your particular plan.

Some plans require prior authorization for specific procedures or restrict which outside services (laboratory or x-ray) can be utilized. It is a good idea to review your insurance benefits prior to your visit. If you do not comply with the requirements of your plan, you may be held personally responsible for your bill. Our staff is available to assist you in these matters.

We offer courtesy billing for nearly all insurance plans if you provide us with a current copy of your insurance card.

Insurance

We accept most commercial insurance plans.  Please check with your insurance carrier to confirm that we are listed by your plan.  We will assess your insurance status at the time of your visit, determine your deductible or co-insurance based on that information, and ask for payment at the time of service.  Payment will also be required for all in-office procedures, medications, or medical devices not covered by insurance. All account balances are due at the time of service. For your convenience, we accept cash, personal checks, and DISCOVER, VISA, or MASTERCARD.

Medicare – we welcome patients with Medicare coverage.

No Insurance? – If you are not using insurance coverage, we require that all initial visits be paid at the time of service.
