Your Visit

We made the difficult decision to close the practice, and as of April 4, 2025 are no longer available for appointments or other care. We are not accepting new patients. Your continued healthcare is important to us. During the next weeks we seek to ensure a smooth transition of your care from Altrix to the provider/practice of your choice.

Medical Records

We will forward your healthcare/medical record to the healthcare provider you select.  Transferring your records is important because the information in your record will help to ensure that your care continues in the most efficient way possible.   

AFTER APRIL 1st:  After April 1, our medical records will be stored by Morgan Records Management.  You may request a transfer of your records from them by completing the Morgan Records Management Release Form. Records may be sent electronically for a fee of $35.00 or a USB Drive can be mailed ($55.00).  Your completed authorization form may be emailed to Medical@MorganRM.com. They will send you an invoice with payment due upon receipt.  OR you can mail your form to Morgan Records Management, 8 State Street, Nashua, NH 03063 with a check for the payment amount based on your selection.  Requests are processed in the order that they are received. Your records will be available for a period of 7 years from the date of closure of the practice.

Payments for Visits

Altrix Primary Care – Nashua offers several payment options for covering the services we provide for you. Option 1 is payment through commercial insurance plans. (See BILLING AND INSURANCE/INSURANCE below). And, of course, you may also pay directly for our services (Option 2). 

For all patients, any copays, coinsurances, deductibles, noncovered services and self-pay balances are now due.

Billing and Insurance

Insurance and billing procedures can often be complex and confusing.

Our nurse practitioners were contracted primary care providers with many insurance plans.

We offer courtesy billing for nearly all insurance plans if you provide us with a current copy of your insurance card.

Insurance

We accept most commercial insurance plans.  We assess your insurance status at the time of your visit, determine your deductible or co-insurance based on that information, and ask for payment at the time of service.  All account balances are due at the time of service. For your convenience, we accept cash, personal checks, and DISCOVER, VISA, or MASTERCARD.

No Insurance? – If you did not have insurance coverage, we require that all visits be paid at the time of service. If you have a balance due, please pay on-line as noted below.
